I feel that this new visa will flounder. It really does little more than some of the others can cover and requires a financial requirement that will put folks off as it will make no sense from a financial perspective. But as this is Thailand... we will have to see what will actually happen and.
Self isolate for 5 days. Retake ATK every other day is recommended. If negative after 5 days... then isolation is over but you must wear a mask for a further 5 days is the recommendation. If still negative repeat isolation and seek medical attention. Certainly seek medical attention if systems get worse and/or you run a fever and/or shortness of breath.
Never done it. And no one gives a hoot. I have asked my local immigration office and they simply say no problem as they know where I live and have done so for years. Besides hotels will... or rather are supposed to... report the foreign guests.

Income differences exist everywhere but what you suggest is that we should feel sorry for being able to afford this or that. As for the US... that has been policy for decades now. Same with the UK and EU with slight differences. Dual pricing is simply a public relationship nightmare. If Thailand wants to recover their tourist industry that equates to some say 25% of their GDP then be fair about it... or scam the tourists out of their money which is what dual pricing is... a scam. Not a good look or enticement for foreigner travelers. Racist? No. Stupid? Yes.

3. Financial qualifications
(a) Applicants must have money deposited in Thai bank located in Thailand with the amount of not less than 3 million Baht; or
(b) Applicants must have money deposited in Thai bank located in Thailand with the amount of not less than 1.8 million Baht and have income with the amount of not less than 1.2 million Baht per year. Once the applicants enter Thailand, they must have accumulated money deposited in Thai bank located in Thailand not less than 3 million Baht within 1 year.
The money in (a) and (b) must be kept in bank account at least 1 year before withdrawing and, within another next year, the money must be left in the account with the amount of not less than 1.5 million Baht and can only be spent in Thailand.
4. Applicants must have no forbidden diseases according to the Ministerial Regulation No. 14 (B.E. 2535) which include Leprosy, Tuberculosis, Elephantiasis, drug addiction and third stage of Syphilis.
5. Applicants must have Thai medical insurance during their stay in Thailand (per the approval of the Office of Insurance Commission) and medical claims for outpatient must not be less than 40,000 Baht, for inpatient must not be less than 400,000 Baht.
