Drivers license, original and copy pasport, medical certificate (normally not needed for after a 5 years drivers licence, but take it anyway) Copy Visa, copy and original of work permit/ place you live/stay/residence

90 days report you can only do online IF it is 14 till 7 days BEFORE date , so the last seven days en before 14 you get error on your screen! Can this be the problem?
