If you travel time is less than 60 days I don't think you will get a tourist visa. It's moot if you can enter visa exempt for the same time frame.

But I would be surprised if they didn't do it for the money.

I have to admit that I was spoiled when I filed in NY. Didn't have to provide 6 months of pay stubs.

Is he a company or an individual? And your vendor statements show money coming from the company or an individual? They have to connect the dots.

You may have one branch that granted a bank account. The reality is its pretty much a dead stop NO with a DTV or tourist visa.

If you make an appointment at Bangkok Hospital via email they will confirm the appointment in writing. They won't give you a list of treatments and dates until you are seen by a doctor.

A few things.

I applied in NY and only provided the Articles of Incorporation which shows me as an officer and my IRS EIN letter. I did not provide any further info other than a letter outlining my name, job title, my responsibilities and my authorization to work remotely as a corporate officer. I provided no further info about my company.

As it regards the bank statement they will probably reject the corp bank statement as they want to see money in your name. Also a screen shot will not be acceptable. It's easy to modify the info on the web page if you know what you are doing. Has to be in your name and should be a PDF. I just supplied a summary from Schwab without transactions.
