I made out my own and had the landlord sign and dated it. She wrote something in Thai. My hotel doesn't have a phone. It went fine. There are hotels that have no phone and don't know how to write. Just do it yourself and have them sign it
a long time ago my girlfriend broke her arm while we were in Nepal. It cost $12 USD to x-rays and cast.. they removed the cast on Ko pipi for less than that. I've had a few doctors in Asia and it costs the same as a pad Thai.
