locked phones are very common in the US. You purchase the phone from the carrier on a payment schedule. When the phone is paid for. They will unlock it and you can choose another carrier

I believe part B is mandatory unless you have other insurance. You will have to show you are covered under another insurance

I’m from Florida and have used the DC embassy before. They’re very slow and will tell you 14 days before traveling. Best way to contact them is thru Facebook message. Keep checking your email in the past they have always asked for additional information.
