Not sure if it is still the same, but when we applied for a TP in November we got the same message and the solution was to add a "space" to our 9-digit passport number.

It says “two adults, one room” so I take it that you do have a double room. Regarding the PCR test: we had the same and I asked them to add that it was one test for each person, which they did. They also specified on the voucher that the payment was for the room, transport and one test per person. problem at all. Often these things are not a matter of deceit….

We flew to Phuket a couple of days ago. We had to show proof of full vaccination at check in at the airport.
