Can anyone shed light on the city of birth part of the application? I'm a U.S. passport holder. My passport biodata page specifically indicates place of birth as the state only. Do I type just the city? The state? Both? I don't want this to be a costly mistake...

I've heard that if any information does not match exactly how your passport appears, you could run into trouble.

I have a US passport and I'm filling out the initial information for the DTV, and I'm already running into problems.

The application on the official website is requesting 'Place of Birth' and 'City of Birth'

Place of Birth is giving me a dropdown menu with a list of countries (so in my case United States)

Unfortunately, on my US passport, it lists a state for my place of birth (in this case, Indiana), and does not list the city in which I was born. So on the website it's asking for the city where I was born, but that's not listed on my passport at all. Should I write 'Indiana' for the 'City of Birth' or should I write the name of the actual city?

Similarly, my US passport does not list a place of issuance, just 'issued by the US Department of State'; I believe it was issued in LA, California where I currently reside.

Has any US DTV applicants ran into any trouble in this regard? I've also heard the Chicago and Washington DC Thai consulates are a good place to apply from, but as I am in California at the moment I think I am stuck with the LA consulate; hopefully that won't be a problem?

For the LA consulate, has anyone who's gotten approved for the DTV, what did you use for the "place of issue" and "city of birth". I've searched around and got mixed answers. Some say for the place of issue use USA, United States, or United States Department of State. For city of birth some say use the state as in the passport and others say both city and state. Should you use city [space] state, ex Seattle Washington or just stick with the state (as seen on the passport) even though it asks for the city? Any advice would be appreciated.

Update: Thanks everyone. For other people's reference this worked for me.

Hi there, I wanted to ask for clarification on two items for the DTV application as an applicant in the US-LA Consulate. I did some searches and couldn't find answers.

1. The DTV application asks for city of birth but my passport lists the state of birth. Which should I use?

2. I have a hyphenated last name. The application does not allow for hyphens. The MRC data at the bottom of the passport has a "<" between my two last names. Do I use a space or run the two last names together?

My DTV visa request was denied at washington DC royal thai embassy because “the information in the form didn’t match my passport”. They wouldn’t give me any details other than that as far as what didn’t match. I checked everything muliple times.

One of the questions on the form is “city of birth”. I put the city but my passport only says “California, USA”. Has anyone been denied for this before and for the next time should I put California for that question so it matches my passport?

Under city of birth, am I putting my actual city, or on my passport it has my "Place of Birth" and my suburb, which isn't a city... Any ideas?

I have a silly question for US passport holders.

The form asks for city of birth but our passports use the state and country. Did anyone get any questions about this? I'm just not sure how they'll verify which city I was born in since nothing I'm submitting will have this info. I'm currently in HCMC and plan to apply here.
