What should I enter for my city of birth and hyphenated last name in the DTV application?

Hi there, I wanted to ask for clarification on two items for the DTV application as an applicant in the US-LA Consulate. I did some searches and couldn't find answers.

1. The DTV application asks for city of birth but my passport lists the state of birth. Which should I use?

2. I have a hyphenated last name. The application does not allow for hyphens. The MRC data at the bottom of the passport has a "<" between my two last names. Do I use a space or run the two last names together?

TLDR : Answer Summary
The user seeks clarification on two aspects of the DTV application: whether to use the state of birth (as per passport) or city of birth, and how to format a hyphenated last name given the application restrictions. Responses suggest to use the state name, include both city and state for birthplace, and to adhere strictly to the name formatting on the passport, avoiding special characters and errors.

Give city and state for birthplace.

Make absolutely certain you use the exact same spelling of your name as on your passport data page. Probably no special characters will be there. Use the same spacing. Check it several times. Error here is an automatic failure.

Also be very careful with the birthdate and expiration date.

The machine will often change it. Again, automatic failure if it’s not correct.

Will do, thank you. My passport has a hyphen in my last name which isn't allowed. The info at the bottom has an arrow between my two last names. My question becomes, do I put a space between the two last names or run it together as one word? One of them is going to get me denied, one isn't. I can't find clear guidance anywhere.

Put the state name exactly as in your passport, I don't believe the name field allows for anything other than English alphabet characters, so add a space.
