For the LA consulate, has anyone who's gotten approved for the DTV, what did you use for the "place of issue" and "city of birth". I've searched around and got mixed answers. Some say for the place of issue use USA, United States, or United States Department of State. For city of birth some say use the state as in the passport and others say both city and state. Should you use city [space] state, ex Seattle Washington or just stick with the state (as seen on the passport) even though it asks for the city? Any advice would be appreciated.
Update: Thanks everyone. For other people's reference this worked for me.
City of Birth: I put down the state as my passport only shows the state
Place of Issue: United States
TLDR : Answer Summary
The question focuses on what to enter for "place of issue" and "city of birth" when filling out the DTV application for the LA consulate. The user received mixed advice from others, some suggesting to use the information exactly as it appears in their passport. Notably, one user successfully indicated just the state for the city of birth, as their passport did not list the city, and noted that they put "United States" for place of issue. The consensus emphasizes that your responses should generally reflect what is on your passport.
