Immigration gave me this form for my visa extension. Support of a Thai child

Its a new form to me, but I want to get it right. As I understand it it's a family member confirming your family.

The first part looks like its filled in by the family member, can this be anyone? Wife? Mother in law?

Signature 2. Competent officer? Imm officer?

Signature 3. Witness? Who can sign?

Getting all the paper work ready to head to Savannakhet. I would like to know if the wife should sign all her papers in thai and or also in english?

Hello all. When completing the visa applications for my children, do I sign my signature on their application or leave it blank?

My daughter is 12 and my son is 7. We are all applying for METV.
