What is the process for completing the Affidavit of a Family Member Form for visa extension in Thailand?

Affidavit of a Family Member Form

Immigration gave me this form for my visa extension. Support of a Thai child

Its a new form to me, but I want to get it right. As I understand it it's a family member confirming your family.

The first part looks like its filled in by the family member, can this be anyone? Wife? Mother in law?

Second part my details?

Third part

Signature 1. Family member

Signature 2. Competent officer? Imm officer?

Signature 3. Witness? Who can sign?

Form STM.10 is filled out by the Thai Citizen Child (or in the viewpoint of the Thai Citizen Child if the Thai Child is not yet of legal age)

if the Thai Child is not yet of legal age, then you as the Foreigner Parent would sign Form STM. 10 on behalf of the Thai Citizen Child, or the Thai Immigration Officer will give instructions for what to do

in Sections 1, 2, and 3, the Name/Details to specify are you as the Foreigner Parent

this Form STM. 10 is mandatory and used by all Thai Immigration Offices across Thailand

I can't answer specifically about this form, but for marriage when they want a witness it is treated differently by office. Some just want a signature and a copy of the ID card and/or house book of the person who signed. Other offices want that person to actually go to the immigration office with you.
