90 days report took me about 3 minutes yesterday. Much more time was spent for driving to the immigration =)

It is free and you should get it by post within 3 weeks. Want here and now? That is not free =)

Honestly, it highly depends on the province. Phuket immigration might ask you anything between 300 and 500 baht. Krabi immigration in most cases do it for free. Other provinces... No idea =)
