The dates on your E visa is the window that you have to use the Visa in other words, you have 90 days to use the visa has nothing to do with your length of stay.
I believe the OP stated that he meaning the agent “put the money in my bank to get me over the top” sounds to me as if the agent supplied the money to qualify him for the visa. 
I think she doesn’t realize that they don’t put that sticker in your passport anymore from the US. She doesn’t appear to know about the E Visa program and I think she posted the photo just as an example. Actually that entire Passport has expired since they’re only good for 10 years.
If none of the above suggestions work, maybe you should try sending them an email and ask them for an example of what they will accept for an income statement.
I do it every year. My extension begins January 31. I leave the end of March and I come back in October. All I do is purchase a reentry permit then of course in December it starts all over again with applying for the next year Ext. As far as your first question at the end of your extension or up to 45 days prior to the Ext expiring, you must reapply for a new extension every year.
