Thailand Plus for Bangkok, nobody asks for it. MorChana for Phuket, they ask if you downloaded it on arrival there.

How long does it take to get a Special Tourist Visa from the Thai embassy in Washington DC, and can I extend it if I arrive on July 3?

My status on the website never changed beyond that so no worries. You should get it all back any day. July 3rd was set as the 'no-extension' date since it's in the last 90 days of the program so you might be screwed staying beyond 30 Sept
