It depends on what embassy you are applying at. I only showed 1 month with the ending balance of the required amount at Washington DC embassy.

you legally can't work in Thailand on any DTV visa, can only work remotely for a non Thai company.

That's why I didn't do the freelancer even though I own my own business. I just got a letter from my doctor I've been seeing in Bangkok and she wrote me a letter stating I'm a patient of hers and the hospital on official hospital letterhead. Was approved with no questions aaked.

I'm not married. I just had a letter from my doctor saying I'm a patient of hers and the hospital, on an official hospital letterhead!!

I applied through Washington DC, I didn't have to show a plane ticket but they did ask for a flight number. I only showed 1 month of bank statement. I rent a condo, so that was the address I used.
