Contact your bank. Recently I could not pay a JAL flight, because there was a block on such transactions.

But honestly we haven't seen any report of people getting denied because of that. What we have seen, is people getting denied because of the passport-place-of-issue field. Travel information have been interpreted differently by different people, but it has not lead to refusals, AFAIK.

It asks for the departure date, not the departure flight. At least that is how I interpreted it. "Last port of embarkation" + "Port of arrival", it seems quite clear to me that it refers to the arrival in Thailand.
