If your o-visa is valid till 5.11.18 then each time you enter the country you get 90 day. Up till the last day of the visa. So in fact, if you enter on 5.11.18 you get stamped in 90 days. This is my experience on a B-visa i'm pretty sure the same rule applies for an O visa. However, the extension you will need to apply before the visa expires.

Get a certificate from the institute that is paying your pension. Then get that translated into Thai and notarised by a Thai lawyer. That should do it. I did this for my dad in Chiang Mai. No problem. We did not need any paperwork from any embassy.
I understand but being misinformed or uninformed could end up very costly. Again, depending your situation. Just happy to share my experience. That's all 😉
