What is the process for obtaining the Affidavit of Income for a retirement extension in Thailand, particularly for Belgian citizens?

Does anyone have the download link to the Affidavit of Income (if that's the correct name?)

It's for my fathers retirement extension (x-amount in the bank and x-amount pension to reach 800k) The Embassy keeps forgetting to email it to me.

TLDR : Answer Summary
The discussion centers on obtaining the Affidavit of Income necessary for a retirement extension in Thailand. The user is trying to find a download link for the document and has had trouble receiving it from the Belgian Embassy. Community members suggest sending a filled affidavit form along with a pension statement by mail, discuss the country-specific nature of these documents, and confirm that the document required differs by nationality.

Je kan het via post, ingevuld opsturen, samen met een pensioen uitreksel en een aan jezelf geadresseerde envelop, 820+40 baht toevoegen. Enkele dagen later heb je het terug
