You can apply online. Keep in mind that you need to apply a few days before travelling as the approval can take up to 5 days.

The price is about 800 thb or €20.

When you're too late and need to apply for a 24hrs fast track, the price can go up to €250

If it's still is as it used to be, you need to get there around 5 AM to get in line for an early queue number which they start handing out at 8:15

Of course there is one and this is the only one you should use in order to be sure you have a genuine and fully refundable ticket. Thai railway offers a 100% refund up to 24 hrs before travel. After that it's 50% and the latest time you can get a refund is 4 hrs before departure time.
