I had my dr write a cover letter explaining the need for everything, the history of previous appointments and then list the frequency of my future visits.
For example
Every week for 6 months
Then every 2 weeks for 6 months
Then every month for 12 months.
I also supplied my medical history and outpatient report alongside those
Yeah I just flew back to the Uk from Thailand but absolutely think with what I supplied I could have just exited Thailand to any country and applied via the E Visa selecting London still. Would have saved me a lot more money but was anxious to risk.
