Going to Thailand on a non immigrant “o” visa, do I require a medical cert, health insurance, foreign insurance, cert of criminal clearance?

Started to fill form in on Thai official website and it asked for all the above. Am I on the right page?

My husband and I are Americans and will go for a visit the end of May. We are looking at visa options to apply for in America while we continue to make the required monthly transfers of our retirement income. One of the options requires a criminal clearance from Thailand and from the US. I have two questions. First, where do we go to get this in Chiang Mai? How long does the process take? The second is, can we get it from the city we maintain residency in the US or does it have to be a national clearance? Thanks!
