Where on the Thai Immigration Site can I find the updated/actual STM2 and Acknowledgement for Overstay Forms?

I was at the Phuket Immigration with all the required forms already filled in and signed (TM7, STM2, Acknowledgement) which I downloaded at the official Site:

The Officers who checked all docs immediately took out my STM2 and Acknowledgement and I had to fill in "new" ones because mine were not the updated ones (?).

Has anyone got the covid extension from Chonburi lately?, could somebody give me a thumbs up on what photocopies i need, i like to to reduce my stress levels by having anything prepared before i go in.

I know before 25th

Got uk passport

Came with tr60

Has been extended (yesterday)

Got tm30

Got tm6

Hello!

Has anyone done the covid extension lately (preferably in Koh Phangan)? I'm slightly confused about what documents are needed and what to list as a "reason".

Also does it depend on the country I'm from?

Has anyone had this rejected?

What docs do UK citizens need for covid extension ?

Realised as an Emirates ticket holder and current covid test requirements it might be best to extend.

Koh Samui immigration update from this morning...

2nd covid extension.

60 days, granted. From the date the current one expires (29/01)... So, until 30/03.

No loop closure required.

Queues not bad. Took a total of 2 hrs (including 1.5hrs where we were told to come back at a certain time, which meant we could leave and go grab a coffee).

No mention anywhere of any possible covid test measures required.

Forms needed:

TM6 copy, all other relevant passport page photocopies.

TM7.

STM2.

Covid application.

Affidavit.

No embassy email stating they are not issuing letters.

No "Acknowledgement of penalties for overstay" form required.

We had to re-fill a couple of the forms as we went with the same format we used in November (helpfully supplied by the admin of this forum ๐Ÿ‘), and they had "changed". (All of the information was exactly the same, it just looked different on the page).

Also, nearly got asked to re photo copy passport pages as they were landscape instead of portrait ๐Ÿ˜‚๐Ÿ˜‚๐Ÿ˜‚๐Ÿ˜‚

Update from Chiang Mai Immigration

I successfully got a 60 day extension due to covid today at Chiang Mai. No under consideration period, fully stamped in to the end of January. I entered Thailand on a Non-O visa for marriage, and already used my 60 day visit family extension which was expiring on Nov 26.

This is what I brought with me

Printed email showing that the Canadian embassy won't issue extension letters

Covid extension request that Tod Daniels posted in this group, but they made me redo this form on an all english version

Acknowledgement of Penalties for Visa Overstay

Passport photocopies for name page, all entry stamps and visas, and TM6

Photos

It took less than an hour from start to finish, no problems whatsoever. Hope this helps
