This can be done online but I haven't done it. My landlord applied for a username/password to use the website, but still hasn't received confirmation of the account after two weeks. There are others online who also say it can take a while to get the online system up and running. However, if you left Bangkok and showed your passport somewhere that registered it, then you should re-register with your Bangkok address before you go to Chaeng Wattana to apply for a new visa using your Bangkok address.
The form I posted is the standard TM30 form, yes. I know that you can self-file at Chaeng Wattana, filling it out as in the photo, because I did it two weeks ago. I don't know the regulations at other Immigration offices, though, and they could be different...This is Thailand, after all.
Two weeks ago, I registered by myself (and paid the fine), without any information from my landlord, using the form linked below. This works at Chaeng Wattana in Bangkok, but I don't know about other offices around the country.

If you use your wife's details to make travel reservations to other provinces, you can reduce the chances that you'll be asked to show your passport, but you may still be asked for your passport upon check-in...in this case, it's up to the whims of the staff at your hotel to look the other way.
Legally, the owner of your condo is responsible for registering you. There is an online system, but this requires that your landlord register with their details (and sometimes it takes weeks just to get a username and password). Legally, this registration must be done 24 hours after you change addresses. Alternatively, if you're traveling with a Thai, you can make all the reservations in the name of your Thai friend and (potentially) not show your passport when checking in...Alternatively, many airbnb hosts will not ask for your passport, so going that route may also save you the hassle.
