When can/should I go to immigration to extend my one-year non-immigrant O visa? The last few times, I've gone 30 days before my visa expired. Can I go two weeks before it expires? Can I go the week before? Is there a hard-and-fast requirement to do this?

I think you're confusing the earliest you can go (which is when you have 30 days or less at most offices) and the latest you can go (which is the day your current stamp expires)
So you can do it any time between those two dates.
And remember some offices allow you to get an extension when you have 45 days or less left on your current stamp
