you don’t have to apply for a visa exempt. You do have to apply for the extension.

If you need more than 75 days you will need to apply for a visa before you arrive to Thailand. You’re able to apply for an extension of 30 days once you’re here on the 45 day visa exempt stamp.

another thing. You can book an appointment if you wish. If you arrived on a visa exempt, you go to the IT Square complex 3rd floor (Laksi) take a left once off of the elevator. I would suggest you have everything photocopied before you arrive. That area is pretty busy.
