ORIGINAL POSTERWe are applying for OA visa. Where do I find medical form which needs signing before we leave UK.
We have two options for flights. We could fly Emirates from Bham or Tha Airways from Heathrow. Would it be better for us if we fly Thai?
Bham is the closest airport.

ORIGINAL POSTERIm in Thailand on a 60 day tourist visa already extended for 30 days. On my return to the UK I what to change to a A/O multi entry visa.
Question is using the medical form printed from Thai Embassy London web site, can I get it filled in / endorsed in a Hospital in Udon Thani and be acceptable for my AO Multi Visa application
