maybe that has changed or they got lucky, but generally first time need to present in person. That's sort of makes sence as they want to see you face to face after that they don't seem to care if done online or in person.

you can definitely try and get an Ed visa, or as others have said get another METV, but that all depends on if they will issue back to back METV.

yes but you can take a lamented copy of your passport, or a copy, you don't need the original, these people don't expect the original passport as the know it's going to get wet.
