JD Gruen, I've downloaded the income Affidavit and filled it out, my plan is to have it notarized at the American embassy outreach that will be here in Phuket on Wednesday, November 15, 2017. I just need to know where the outreach will be held. There is no information on the website.

Thank you for your advice, I agree with you. I looked on the American embassy's website and there is an outreach here in Phuket on PHUKET: Wednesday, November 15, 2017, but it doesn't say where it will be held. Does anyone know or should I call the embassy?

I hate to be a bother, but you mentioned that I need a notarized copy of my passport, what about the document from Cal-Pers (my retirement) does it need to be notarized as well? and what about bank statements (which I don't have) although I can go online and show my accounts, will that fly?
