Same as every visa... the validity (5y) starts with the day you apply (or the approval). The permission to stay (180d) starts with your entry.

They will correct it, not a big problem, but you probably have to go back to the point of entry. But give your local Immigration a try first...

This is from Bangkok, right? Looks totally different from the TM7 which is normally the form for the retirement extension.

"...I have guaranteed...", "...made an oath..."? I've extended O and O/A many times already, but I have no idea what this form is for, never seen it before...

Nope, it probably won't, at the moment it's postponed. And even if it would start as a pilot with Australians for some month.

That would be 129 days... so... just arrive on visa exempt (60 days), plus one extension (30 days), one border bounce to get again 60 days.
