I haven’t done it last time I was here , arrived in late August, flying out later this week, and will be back late October again. Any problem or fines I have to pay while flying out ?

As I am new to this , can you also kindly let me know what exactly I need to submit to authorities? Is it the rental contract, copy of passport, visa and stamp page and TM 30 form? And where do I get this form? From the office?
