I got a future appointment slip too and an itemized invoice from the hospital plus that letter with the diagnosis and treatment plan. I'll send that through sounds like it should be enough

I don't use NAB but I do have an account so I just went and had a look and that's perfect. The requirement only says financial evidence of the funds so that'll do the job. You've just saved me a long ass wait lol thanks heaps legend. I'm on holiday in TH now so I'll need to wait until to get home to apply I'm pretty sure I've read
