Somewhere on any visa the word "single" or "multiple" should appear. If it says "single", then re-entry permit is required

This should also tell you the order in which they are required.

Lee Brookes my friend works at the embassy in London and told us it's about £30 - £50 for a certificate. My spouse ended up not going, but We even found one who does it by phone and emails a certificate to print. All acceptable to the embassy. Gogodocs.
