Is this actually a government mandate issued for all ASQ's? Looks more like something that a particularly risk averse lawyer would draw up for individual hotel(s) wanting to cover their backsides.
I sent exactly the same as you are planning to do. I thought I'd get an email if they required further doc's. After a week with no email I posted all the stuff you have listed only to receive an email requesting further docs (eg, declaration, copy wife's passport and i/d, marriage cert and village book). Personally, I would suggest simply sending what you have listed initially (after all, that's all that they ask for) and then wait to see if they request further details.
