The doctor will definitely need to write a medical certificate stating your chronic disease etc.
They will probably call the clinic to check, so best is to prepare them for the call , so the doctor can tell them you are an actual patient. You don't want some incompetent receptionist answer the call and telling them you are not a patient.
Documents: Receipts, medical certificate, letter of doctor, treatment plan, appointment card, and if possible document proving the clinic is real.
You should have put investor instead of unemployed. And then maybe you could show your crypto transactions etc. However, in Taiwan they told me crypto investing is not real.
