Yes the embassy will provide you with a form to fill in and sign for the immigration

And the thai immigration will also give you an form to fill in just take the forms and both of the passports and they'll do the rest

 now I've been asked for My contract for the condo every year including a copy of the blue book and the id owner and a letter confirming that I live in the condo for the last 5 years including the tm30
