You say you are currently in Thailand so contact an established hospital and make an appointment so they can assess your condition and provide a treatment plan that is of at least 6 months duration signed off by a recognised doctor at the hospital. Once you have this you will have to leave Thailand in order to apply for the DTV at a Thai consulate of your choosing.

you seem determined to ignore the options available to you. If it takes half a year to finalise the process get a 90 day Non-O and get it started. If it's not finalised in that time leave and get another 90 day Non-O and return. I really don't understand why you have a problem with this
he's not married and under 50 but has a Thai kid and is named on the birth certificate. But he doesn't want to go through the process of obtaining parental rights for some reason. So he can get a 90 day Non-O visa but can't get the extension of stay
I'd say it depends on what type of income you have. If you have income from work you will do remotely from Thailand you should use that account BUT you then need to apply using workation and not soft power. If your income is passive then no problem
