Try calling it address report instead and they should know what it is. It's illegal for hotels to have foreigners staying with them without reporting it to immigration.

I just got my TM30 without a TM6. Just leave the field empty.

I've never used an agency to do a real visa run, only border bounces, so I can't answer that.

But yeah, being in Thailand "too long" as a tourist could be a reason for denial.

no, the day of collection they only had one guy sitting in the counter handing out the passports and refusing to talk to you because so many people were waiting for their passports.

2 other people got denied too and none of us got an explanation. That's normal in most countries, but it was the fact that they put a visa in my passport and stamped it with a denied-text that was my issue.
