 Visa exempt is one thing, the TDAC is another. You do the TDAC within 72 hours of arrival and when you arrive you get visa exempt stamp for 60 days they are not linked together.

Do fast track services at Don Mueang Airport actually speed up immigration processing?

Quicker but waste of money if you have checked luggage as you still have to wait for it same as everyone else.

What are the current visa extension rules and tourist visa options for Thailand?

A tourist visa is not 90 days it only 60 days then you need to try get the extension to give you the extra 30 days.

The first extension on first entry is 30 days and the extension on 2nd entry is apparently only 7 days

 Yes and many Agents lie to get you to buy something else. Many Agents tell different versions also
