The guy at the copyshop outside of immigration knows and you can just give him your passport. Don't bring in your own forms like someone suggested. I did this but it wasn't exactly like the 1 they had so they made me fill out theirs instead. You won't need any paper copy of a tm30 application either. Just make sure where you are staying has registered you with immigration before you go. Samui is the easiest place in the country to get covid extensions. No under consideration period, 1900 baht. In and out in an hour.

Lots of incorrect information here. You don't need to be on a long term visa or be old or sick to get the vaccine. I'm 33 with no health conditions and on a covid extension and I was vaccinated here with AstraZeneca.
