The extension of stay based on medical purposes is sold up to 90 day increments. Just because an immigration officer isn't happy issuing it doesn't mean anything. You're out of nothing applying again.

Can I extend my Special Tourist Visa (STV) in Thailand after 270 days?

The only way to stay longer is with an Extension of Stay based on Unable to Travel due to COVID-19. This is an optional extension of stay, and the immigration office does not need to grant it.
