Once you have done the printing of the bank statement you will also need to take the statement to your bank and then they will have to stamp the statement with their bank stamp
Or the immigration office will not except it without the bank stamp as I had the same problem last year
I was told that only the retirement visa application was made to show the bank statement even though its not on the list of things that we must show them
ok then once you have gone to the new passport office with your form and paid for the passport
And your new passport has arrived you will be given a form from the passport office for the thai immigration and you will need one for the thai immigration office so that is two forms to fill in and you will need a copy of the old and new passport's to give the immigration.
And if its a retirement visa application then they will ask you for your savings book and the full years statement from the bank
But make sure that you know how long the bank will take to process it for you
