Thanks for the information. We're in FL. Got our vax at Walgreen, who also registers it with the FL Dept of Health. Neither of those make QR codes for vaccine proof. So, googled it up and seems can get a QR code from CDC. Will be doing this ASAP! Thanks for the heads up. This is getting to be a pain... We've moved states since getting a OA visa in 2018. My Doc here wanted me to get a syphilis & tb test. My Doc back in NC, just looked at the form, laughed, and signed.... Hoping to get the Doctor to sign form tomorrow

Thanks for the information. We're in Central West FL. Just went to Walgreens a couple days ago. Was told from 2 to 5 days for results. When I explained the circumstances - I got a shoulder shrug...

Yes, but have to get the medical form, & police background check, correct? (working on OA & O visas for me and wife now.)

Yes, guess it appears so. Just thought it would prolly want the financial/medical/criminal information first, since the TP has been added - as I think it requires the flight/insurance info too. Will be proceeding... Stay tuned - lol.

Thanks! I just remembered the age limit was higher. Just a few more weeks, will be applying for a OA visa... Can't wait to get back!

AXA states "Age: 6 months - 74 years upon the policy effective date". Have you tried SafetyWing, or maybe Pacific Cross?
