...by the way, just received a report from an STV extension done today in Chiang Mai - no insurance, no accomodation asked (no flight ticket as well). And he was told he can apply for an extension again until latest September 30th and stay until December.

yes, but the last sentence on the bottom right makes me thinking - sounds like no further extension possible if your current stamp exceeds September 30th

you can book your insurance with start date on entry (or at least alter the start date once you know your entry date), but yes - they print in the validity dates of your insurance. Afaik you do not loose time of your entry stamp, but need to prove your insurance again.

Anyways, something you don't want to deal with if you can avoid it upfront easily.

you CAN appky for CoE weeks in advance, but honestly I would apply around 10 days prior departure. If something changes (i.e. flight cancelled), you have to have your CoE changed. At the moment you can not do that online yourself (nor simply apply for a new one), so you have to contact your embassy directly and ask for a change.

Turnaround time for CoE is quite fast, often only hours. But in the USA they wait until 3 days before your departure to issue it (to avoid changes after issuance).

the CoE is issued for a specific flight on a specific date and a specific ASQ. There is no validity time frame, only valid for this specific itinerary.
