Jim, I had an issue with the middle name at LA Thai Consulate 555. When I applied, it had a place to put my middle name and I used it. My Visa was denied because it did not match my passport. I re-applied adding my middle name in the first name spot (i.e Kevin Middle name) It was approved the next day. Just make sure that it matches how your passport lists your first and middle name. That was an $80 mistake LOL 555.

I just received my Non-O visa for marriage from US and they asked for that. I don't think they would ask for that for the retirement though. Make sure you selected the retirement from dropdown menu.
