What should I do if the Thai Consulate website doesn't have a field for middle names on my Non-O retirement visa application?

I'm trying to use the website of the Thai Consulate in Los Angeles, California to apply for a 90 day Non-O retirement visa. I have questions (eg, why is there no field for middle names? Is this going to result in denial?) And the only way to contact this Consulate is with the Line app but the app link is broken..😵‍💫 Anyone else having these problems?
