 Usually a residence certificate from Immigration, passport with a long term visa and a Thai sim card registered against your passport.

What should I know about filling out the TDAC for Thailand?
Graham ******
Put in a date within 3 days then update it daily until you get your arrival date accepted. You only look at the health part if you have been in a different country from where you are departing from within the last 2 weeks.

 The Non-O Visa is valid for 90 days from the date of issue and gives you a 90 day entry stamp when you enter within those 90 days
