My Thai wife’s Coe request was preapproved and then we needed to include the travel and asq info. After entering the travel info, the Coe was rejected because the last port before port of arrival, and the port of arrival were not inputted. I have been looking on the Coe form all morning to find out where to input these two items. Any ideas?

I am on a METV Visa . Did a border bounce to Lao for 60 day stamp . Now submitting an application for 30 day extension . Form ask port of arrival and date . Is this from my bounce into Lao or from my original entry into Thailand at Bangkok on METV Visa . Trying to complete form before I go to immigration.
