How can I correctly input the last port and port of arrival on the CoE form for Thailand?

My Thai wife’s Coe request was preapproved and then we needed to include the travel and asq info. After entering the travel info, the Coe was rejected because the last port before port of arrival, and the port of arrival were not inputted. I have been looking on the Coe form all morning to find out where to input these two items. Any ideas?

It says so right in the COE form. What exactly are you facing issues with? You put your departing airport in "first port of departure" and as "port of arrival" you put your arrival airport in Thailand, so either Bangkok or Phuket, I guess. If it is NOT a nonstop flight and you are transitting at some airport in between, you put this one under "last port before arrival in Thailand".

is your form for Thais or non Thais? My wife’s form does not ask for port names after “final flight number to enter Thailand”. After entering the flight number, it skips to “if you do not travel with direct flight….”. I don’t know why.
