Updated & re-submitted: I changed my contract to specifically say "Employment Contract", not "Service Contract" and had my client re-sign it an update it.

I paid 50 cents to download a certificate of organization PDF of my client's company from the secretary of state website & resubmitted this + the same EIN letter
