they use fingerprints now so a change of passport probably won’t work unless it was a long time ago

There is no official fee for a residence certificate. It’s supposed to be free

That’s correct, you have to cross a border and back every 90 days. No option to do a 90 day report as you would with an extension.

, I was of the belief that if your original visa was O-A, that any extension thereafter (retirement or marriage) would require health insurance? Have I got this wrong?

go any talk to or call the Thai consulate in Melbourne. They will tell you exactly what you need for a 12 month multi entry visa
